Requirement: Confirmed Appointment  2 copies of Special Power of Attorney provided by the bank. 2 copies of applicant's Passport. Print the confirmation of appointment with QR code sent to you via email. Step 3.  Information Counter On the date of appointment, present your confirmed appointment to the officer at the information counter & secure a token number to be provided by the officer.  On a side note, a voter's registration form will also be provided by the officer if you are not yet registered.
