Certification, Authentication and Verification (CAV) Application of Academic Records for Red Ribbon

Hi guys!

I had secured my DFA Authentication Certificate or Red Ribbon for my Diploma, TOR and Certificate of Graduation recently. That's my first time so I decided to write it down and share with you. In addition, it will be my future reference in case I need to get one again.

There are three entities involved in the process namely; the School/University where you graduated, CHED, and DFA.

• So first, I went to my school, Saint Louis University Baguio City, all the way from Makati for my CAV application. The entire process (Step 1,2 & 3 below) took me for less than 30 minutes so I lingered for a while and went back home afterwards.
• Secondly, the CHED part is not a problem. SLU took the responsibility of coordinating my application with CHED. Cool! right?! Yeah!
• Lastly, I went to DFA to claim my Red Ribbon documents.

 ...so let's get down to details.


|REQUIREMENTS:

Original & Photocopy of the Following:

1. Diploma
2. Transcript of records
3. Certificate of Graduation


|PROCEDURE:

  STEP 1:

• Proceed to registrar's office and look for the window with the first letter of your surname.
• Present the requirements & fill up the clearance forms that will be given to you
• Note: If your TOR has the stamp "FOR BOARD EXAMINATION PURPOSES ONLY" just like mine, don't worry. Ask the photocopy personnel to cover it before photocopying. 

  STEP 2:

• Pay for the required fees  at the cashier

        Authentication & Certification fees:  PHP 132.00

        CHED fees:                                       PHP   80.00

  STEP 3:

• Return the clearance form together with the receipts to the same window at registrar office.
• Receive your claim slip.
• Please note that all the original documents will be given back to you. So check your documents before leaving.

  STEP 4:

• After 4 working days upon application, go to the website hereunder

            http://www.chedcarcav2dfa.jigsy.com/

• click the latest month found on the upper tab of the website and look for your name 
• If your name is on the list, copy the following to your claim stub;

                     Houseway Bill No.

                     Control No.

                     Date of release

  STEP 5:

• Claim your Red Ribbon documents as scheduled at the 

                 Authentication Division, Aseana Building, corner Bradco Ave., 

                 Macapagal Blvd. Parañaque City.
